AYSO Section Eight 24-Hour Field Incident Report

The Field Incident Report is used to document any game problems or incidents of misconduct.  This report will be sent to the Regional Commissioner and Area Director

The Field Incident Report must be filed:

  1. If a Player's sportsmanship conduct fails to meet the AYSO goals and objectives.  For Under 14 players, this includes whenever a player has been Cautioned or Sent-off (yellow or red card infractions of the laws of the game).
  2. If a coach, parent, volunteer, spectator, or other person associated with the game, exhibits inappropriate behavior during (or before or after) the game.
  3. If a game is suspended for any reason.
  4. For any field problem (field conditions, sprinklers, goal net, etc.).
  5. Or for any other reason,  that in your opinion, may warrant a Field Incident Report.

This report must be filed within 24 hours of the incident.
